Successful Management of a case with Jaundice

A 44 years old male patient presented with itching and right hypochondrial pain, On examination there was jaundice, right hypochondrial discomfort, No organomegaly detected.
Ultrasonography and Liver function tests were requested
US revealed extrahepatic biliary obstruction with enlarged pancreatic head.
CA 19-9 and CT abdomen were requested
CT revealed the same findings as US with enlarged dyshomogenous pancreas, yet no definite Focal lesions.
CA 19-9 was normal, ERCP were recommended, it showed markedly dilated intra- and extra-hepatic biliary radices with long (2cm) lower end CBD stricture suggestive of pancreatic head mass, Stent insertion was done.
The decision of surgery was taken on exploration, the whole pancreas was found to be enlarged and firm with a possible hard mass in the head, Whipple’s operation was done.
The specimen was send for Histopathological assessment
Patient was discharged in a good general condition to be followed up with Dr. Ali Abdul Aleem, Oncology Consultant for chemotherapy.
